If your child has asthma, please have your physician complete this form and return it to the school nurse. An updated asthma action plan along with a prescription medication form are required at the start of each school year.
If your child has food or sting allergies, please have your physician complete this form and return it to the school nurse. An updated allergy action plan, along with a prescription medication form are required at the start of each school year.
If your child has a history of seizures, please have your physician complete this form and return it to the school nurse. An updated seizure action plan, along with a prescription medication form are required at the start of each school year.
Please fill out and return the following document to the school nurse if your child needs to take any over-the-counter medication while at school. This form must be signed by a physician.
If your child takes prescription medication at school, please fill out the parent section of this form and have your physician complete the physician portion. A new form is required each school year and whenever a medication change is made.
