Accommodations for Students with Special Dietary Needs
Program regulations for the National School Lunch Program and the School Breakfast Program require meal substitutions at no additional cost for students who meet the necessary requirements. To request accommodations for students with special dietary needs, fill out the appropriate form below and turn it in to the OSD Child Nutrition Department at 200 Bolt Boulevard, Oxford, MS 38655. Forms for Students with Special Dietary Needs

Special Dietary Needs for Disabled Students
This form is for substantive food accommodations such as special preparation (fine chopping or liquifying) and/or requirement for unique food preparation equipment. Part I must be completed by the school; Part II must be completed by a physician.

Special Dietary Needs for Non-Disabled Students
This form is used for most common dietary needs including allergies (although some allergies can be classified by a physician as disabling). Part I must be completed by the school; Part II must be completed by a medical authority such as a nurse or physician.

Part I must be completed by the school; Part II must be completed by an appropriate religious authority.
